Standardized Permission Slip: Coaches can use this form as a permission form for trips to Texas Quiz Bowl events.
Roster Form: This form needs to be submitted for each team at every tournament.
Student Data Form: Each student who participates at a Texas Quiz Bowl event must complete a student data form. This form allows us to track students for the purposes of monitoring eligibility and selecting an All-State team.
Pizza Form: Students can pre-order pizza at tournaments. They will need to turn in this form to the registration desk with money on tournament morning. In general we only accept pre-orders. Students cannot purchase pizza at lunch without a pre-order.
